Job description

Overview

Promotes optimal patient outcomes by assisting with medication reconciliation. Interacts with patients and reviews medication‑related documentation for the purpose of compiling a medication history for patients being admitted to the hospital. Compiled history information is entered into the patient’s electronic health record to facilitate medication reconciliation by the admitting provider.

Performs a variety of functions that impact the medication reconciliation process and patient care. Seeks assistance or clarification from a pharmacist with any questions or issues that may arise.

Qualifications

Education

High school diploma or GED required.

Registration

Must be registered with the Alabama State Board of Pharmacy as a Pharmacy Technician within 30 days of hire date, and registration must be in good standing “active.” Employees must have national certification with PTCB (Pharmacy Technician Certification Board) or ExCPT (Exam for the Certification of Pharmacy Technicians) upon hire.

Experience

Experience applying customer service behaviors and communication skills required.

Minimum of 6 months of current Pharmacy Tech experience is required. Combination of both inpatient and outpatient experience preferred, particularly outpatient/retail experience.
