Qualifications

• Active Pharmacy Technician registration/licensure as required by state law.
• National certification preferred (CPhT).
• Minimum one year of pharmacy technician experience in retail, managed care, ambulatory care, or population health.
• Knowledge of medication adherence concepts and pharmacy workflows.
• Proficient with electronic health records and pharmacy management systems.
• Strong communication and customer service skills.
• Experience supporting Medicare Advantage or CMS Star Rating programs.
• Experience with population health, quality improvement, or care management initiatives.
• Familiarity with medication therapy management (MTM) programs.
• Experience with Microsoft Excel functions.

Responsibilities

• Under the direction of the Clinical Pharmacist, support the pharmacist in managing medication adherence initiatives and pharmacy quality measures.
• Identify patients at risk for non-adherence and conduct outreach.
• Coordinate medication access programs, copay assistance, and patient assistance resources.
• Document interventions and assist with quality improvement activities.
• Collaborate with pharmacists, providers, care managers, health plans, and patients to close medication adherence gaps.
