Job summary

Sentara Virginia Beach General Hospital is seeking a Medication History Pharmacy Technician II in Virginia Beach, VA. The role involves obtaining medication histories in the Emergency Department and coordinating with patients and healthcare providers under pharmacist supervision.

The position is full-time (40 hours/week) with rotating shifts, including two weekend shifts per month and holiday rotation. Certification/licensing requirements apply, and experience in ED medication history is

Qualifications

• High School Diploma or Equivalent.
• Virginia Pharmacy Tech License (Required).
• CPhT License (NHA or PTCB) (Required).
• 2 Years of Pharmacy Experience (Required).
• Emergency Department Medication History Experience (Preferred).

Responsibilities

• Work in ED to get medication history from patients.
• Discuss with patient or advocate what meds the patient is currently taking.
• Call pharmacies, PCPs, and nursing homes to gather additional med history as needed.
• May also work in central Pharmacy (filling meds, delivery of meds).
• Participates in the training of new employees.
• Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner.
• Ensures adequate supplies are available for continuous patient care.
• Triages and escalates any questions or issues outside the pharmacist technician scope.
• Uses appropriate systems for processing patient information.
• Ensures quality assurance is maintained per department and legal requirements.
• Operates pharmacy equipment appropriately.
• Participates in clinical initiatives to continuously improve patient care.
• Training in an advanced Clinical Pharmacy Technician function.
• Other duties may be assigned.

Status: Full Time (40 Hours/wk)

Shift: Rotating

Two weekend shifts per month & Holiday Rotation required

The Pharmacy Tech II- Clinical performs the technician responsibilities of clinical pharmacy care delivery under the direct supervision of a licensed pharmacist or licensed independent practitioner.

• Work in ED to get medication history from patients


• Discuss with patient or anyone with patient what meds patient currently is on


• Call pharmacies, PCP, Nursing home to gather additional med history if needed


• May also work in central Pharmacy (Filling meds, delivery of meds)


• Participates in the training of new employees
