Baptist Health in New Albany, IN is seeking a Pharmacy Medication Reconciliation Technician who is a certified pharmacy technician and can support inpatient pharmacy services by obtaining medication and allergy histories for admitted patients.
You will interview patients, families, call pharmacies and facilities, and relay information to pharmacists to help determine treatment plans during hospitalization and at discharge.
Summary Job Description: The pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
