Baptist Health Lexington is seeking a Pharmacist for first and second shifts in its hospital pharmacy. The position supports a decentralized clinical services model and offers a $15,000 sign-on bonus for qualified individuals.

Responsibilities include reviewing orders, ensuring timely drug availability, and participating in anticoagulation management and other clinical services, with ongoing education funding and leadership support from Baptist Health.

Formación

• Graduate of accredited College of Pharmacy (PharmD or equivalent).
• Kentucky Board of Pharmacy license in good standing.
• 1–3 years related work experience or completion of ASHP-accredited pharmacy residency preferred.
• Basic Life Support (BLS) certification required.
• If not already certified, must obtain BLS within 30 days of hire date.

• Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, interactions, contraindications and adverse events for all ages.
• Ensures drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive outcomes.
• Provides comprehensive decentralized or pharmacy clinical services including anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support.

Pharmacist First and Second Shifts Hospital Pharmacy I Lexington, KY Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.

Description

Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served. Ensure drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome. Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services. Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patients for discharge.

Requirements

• Graduate of accredited College of Pharmacy.
• Kentucky Board of Pharmacy license in good standing as applicable to practice site.
• 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
• Basic Life Support (BLS) certification is required.
