Job summary

Evergreen Nephrology is seeking a Pharmacy Care Coordinator to launch and run our pharmacy program. You will connect patients with a mail-order pharmacy, manage multi-dose pill packs, and coordinate proactive refill management and care team check-ins.

Based onsite at partner practices in Minnesota, you will guide enrollment, document consent, and support patients through the transition to coordinated pharmacy services and electronic prescriptions.

Qualifications

• Current Medical Assistant certification (CMA/RMA) is required.
• Patient-facing healthcare experience in clinical, pharmacy, care coordination, or patient support setting.
• Comfort and proficiency with an electronic health record (EHR/EMR).
• Strong interpersonal and communication skills to engage patients in person and over the phone.
• Onsite work at practice locations with reliable transportation and HIPAA privacy awareness.

Responsibilities

• Serve as onsite Evergreen representative at practice sites to explain the service and assist enrollment.
• Engage patients with approved messaging to build trust and inform enrollment decisions.
• Obtain and document patient consent and enrollment information.
• Update the EHR with patient decisions and trigger electronic prescriptions to the pharmacy partner.
• Conduct telephonic outreach to introduce the service and complete enrollment or document declines.
• Document all patient interactions and enrollment activities for accuracy and tracking.
• Coordinate between patients, practice staff, and pharmacy partner to ensure smooth handoffs.
• Uphold HIPAA privacy and confidentiality in all activities.
