Evergreen Nephrology, LLC is seeking a Pharmacy Care Coordinator to launch and manage our pharmacy program for patients with chronic kidney disease. You will engage patients onsite and by phone, obtain consent, and enroll them in a mail-order, multi-dose packaging service coordinated with the pharmacy partner.

You will document enrollment, update the electronic health record, and maintain privacy standards while coordinating with providers, staff, and the pharmacy partner.

Qualifications

• Current Medical Assistant certification (CMA/RMA).
• Patient-facing healthcare experience in a clinical, pharmacy, care coordination, or patient support setting.
• Proficiency with an electronic health record (EHR/EMR).
• Strong interpersonal and communication skills to build trust with patients in person and by phone.
• Ability to work onsite at practice locations with reliable transportation.
• Working knowledge of HIPAA and protection of patient privacy (PHI).
• MS Office Suite including Outlook and Teams proficiency.
• Ability to operate in a primarily remote environment with home internet meeting speed requirements.

Your Role

As a Pharmacy Care Coordinator with Evergreen, you play a critical patient-facing and operational role in launching and running our pharmacy program. This program connects patients with a specialized mail-order pharmacy that provides pre-sorted, multi-dose medication packaging (pill packs) along with proactive refill management and periodic care team check-ins, helping ensure patients stay on track with complex medication schedules. Working onsite at partner practices — and by phone when needed — you are often the first-person patients meet from the pharmacy care team. You explain the value of the service, answer questions, provide patient collateral, and help patients feel confident enrolling.

You work closely with providers, clinical staff, and our pharmacy partner to engage patients, capture consent, and set up timely access to medications and pharmacist-led clinical services. You'll explain how the service works — multi-dose pill packs organized by date/time, home delivery and automatic refill coordination, so patients never run out, and scheduled check-in calls to review adherence and address new prescriptions or dose changes. When a patient agrees to participate, you act directly in the electronic health record — updating the patient's preferred pharmacy, triggering electronic prescriptions, and documenting consent or decline based on the patient's decision. For patients who do not have an office visit, or who cannot be supported onsite, you conduct telephonic outreach to introduce the service and complete enrollment.

This role is focused on onsite and telephonic patient engagement, consent and enrollment, EHR documentation, and care coordination for patients living with chronic kidney disease and other complex chronic conditions. You establish trust, explain the value of pharmacy services, and remove the barriers that keep patients from staying consistent with their medications.

Role Responsibilities

• Serve as the onsite Evergreen representative at practice sites, meeting patients after the provider introduces the pharmacy service to explain the offering, answer questions, and provide co-branded patient collateral.


• Engage patients in a clear, standardized way — using approved messaging and supporting materials to build trust and drive consistent, informed enrollment decisions.


• Obtain and document patient consent, and capture the information needed to enroll the patient


• Make the necessary changes in the EHR based on the patient's decisions — updating the patient's preferred pharmacy, triggering electronic prescriptions (eRxs) to the pharmacy partner, and accurately documenting consent or decline in the patient's chart.


• Conduct telephonic outreach to patients who do not have an office visit, or who cannot be supported onsite — introducing the service, establishing rapport, and completing enrollment or documenting declines.


• Accurately document all patient interactions, outreach attempts, accepts/declines, and coordination activities in Evergreen's clinical and operational systems, supporting accurate enrollment and success-rate tracking.


• Serve as a liaison between patients, practice/provider staff, and the pharmacy partner, ensuring smooth handoffs and continuity of care after enrollment.


• Uphold patient privacy and confidentiality standards in accordance with HIPAA and organizational policies.


• Other duties consistent with this role, as assigned.

- This in-person position is located at one of our practice partner office locations in Brooklyn Center, Minnesota

Required Qualifications

• Current Medical Assistant certification (CMA/RMA).

• Comfort and proficiency working within an electronic health record (EHR/EMR).


• Strong interpersonal and communication skills, with the ability to build trust and engage patients both in person and over the phone.


• Ability to work onsite at practice locations, with reliable transportation as required.


• Working knowledge of HIPAA and a demonstrated commitment to protecting patient privacy and PHI.


• Intermediate skills with MS Office Suite of products including Outlook and Teams


• Able to work effectively in a primarily remote environment: Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended


• Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role


• Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

Preferred Qualifications

• Experience with chronic kidney disease, nephrology, or complex chronic care populations.


• Bilingual (English/Spanish).


• Experience with point-of-service enrollment, consent capture, or tablet-based intake workflows.


• Experience in a telephonic or call-center–based outreach environment.


• Background in care navigation, including connecting patients to community resources and addressing social determinants of health.


• Familiarity with common chronic disease medications and prescription coverage terminology.

Compensation

The pay range for this role is $23-$25 per hour. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
