Description

MHALA is among the largest and most comprehensive nonprofit mental health agencies in Los Angeles County. The agency supports over 19,000 low-income and no-income individuals annually with integrated services. MHALA impacts mental and physical healthcare, homelessness and housing, supported employment, and wellness and financial services, with special programs for Veterans and transition-age youth.

Position Overview

Personal Service Coordinator/Intensive Case Manager

35 hours/week / $24.00/hour (salary may vary based on experience and other factors)

Summary: Provide high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the DHS Housing for Health (HFH) program, using a client-centered approach and excellent customer service, sensitive to the challenges faced by people experiencing homelessness with medical and behavioral health issues as they move into and maintain permanent supportive housing.

Essential Duties and Responsibilities
  • Assist clients throughout the housing stabilization process, including completing all related documentation and serving as a contact for LACDA/HACLA subsidies.
  • Conduct outreach and engagement, including processing referrals, establishing rapport, conducting face-to-face screenings, obtaining consents, and linking to services.
  • Handle intake and enrollment activities, assisting clients with documentation, completing forms, and enrolling into ICMS, including temporary housing assistance.
  • Support clients at all stages of move-in, including document preparation, application assistance, coordinating move-in, and tenant orientation.
  • Develop and implement individualized case management plans based on comprehensive assessments, addressing client needs and goals.
  • Link clients to health, mental health, and substance use services, helping establish medical homes and maintain treatment regimens.
  • Assist with income benefits, employment, life skills, community participation, and addressing barriers to housing, including transportation support.
  • Provide housing location services, educate tenants on rights and responsibilities, and assist with eviction prevention and resolving housing issues.
  • Ensure documentation standards are met and records are maintained accurately in Electronic Health Records.
  • Participate in a 24/7 crisis prevention on-call rotation, ensuring clients are aware of crisis resources.
  • Engage in ongoing professional development activities to enhance skills.
Other Position Requirements
  • High school diploma or GED; 2 years' experience in psychosocial rehab services preferred.
  • Ability to work in a team-oriented environment.
  • Proficiency in Microsoft Office applications.
  • Ability to learn HMIS and Electronic Health Records systems.
  • Experience working with clients with mental illness, chronic health issues, and substance use disorders.
  • Knowledge in homelessness, outreach, case management, mental health, crisis intervention, and housing rights.
  • Flexible schedule, including some nights, weekends, and holidays.
  • Valid driver’s license, vehicle, insurance, and ability to be insured by MHALA are required.
  • TB clearance and annual retesting are required.
