Job Overview

Mental Health America of Los Angeles (MHALA) has been a pioneer in mental health services, advocacy, and training for nearly a century. MHALA supports over 19,000 low-income and no-income individuals annually through integrated services including healthcare, housing, employment, and wellness programs.

Position Details

Title: Personal Service Coordinator/Intensive Case Manager

Hours: 35 hours/week

Pay: $24.00/hour (based on experience)

This role involves providing high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the DHS Housing for Health program, focusing on a client-centered approach and excellent customer service.

Essential Duties and Responsibilities
  • Assist clients throughout the housing stabilization process, including documentation, interviews, and subsidy management.
  • Conduct outreach, engagement, screening, and intake activities, establishing rapport and linking clients to needed services.
  • Support clients in moving into permanent housing, including paperwork, orientation, and connecting with services.
  • Develop and implement individualized case management plans addressing clients' needs and goals.
  • Ensure clients access health, mental health, and substance use services, maintaining continuity of care.
  • Assist clients with income benefits, employment, life skills, and addressing barriers to housing.
  • Provide housing location services, tenant rights education, and eviction prevention support.
  • Maintain accurate documentation in Electronic Health Records and participate in on-call crisis prevention rotation.
  • Engage in professional development activities to enhance skills.
Requirements
  • High school diploma or GED; 2+ years experience in psychosocial rehab services preferred.
  • Proficiency in Microsoft Office and ability to learn HMIS and EHR systems.
  • Experience working with clients with mental health, chronic health issues, and substance use disorders.
  • Knowledge of homelessness, outreach, case management, and housing rights.
  • Flexible schedule including nights, weekends, and holidays.
  • Valid driver’s license, vehicle, insurance, and TB clearance required.
