Summary
The Downtown Women’s Center seeks a full-time, field-based Case Manager – Intensive Case Management Services (ICMS) to provide housing placement and individualized case management services to homeless women enrolled in the Housing for Health program supported by the Los Angeles County Department of Health Services (DHS). The Housing for Health program serves individuals who have experienced long-term homelessness, a history of trauma, and have long-standing health and mental health issues. The Case Manager - ICMS will engage women in services, establish housing linkages and placement, and provide individualized case management through the implementation of an evidence-based practice called Critical Time Intervention (CTI). This position is part of an interdisciplinary team of staff to provide field- based services throughout Los Angeles County. As a Case Manager - ICMS, this position will help women achieve housing stability and establish enduring support systems. This work will further inform recommendations and research for other agencies seeking to implement CTI as a solution to ending homelessness among various populations.
Essential Functions
Provide individualized field-based Intensive Case Management Services to participants, including transportation; (on-site and off-site)
•Create individual service plans that ensure housing stability, address each client’s needs and goals, identify and provide needed resources and referrals.
•Assist clients in securing housing, basic needs, medical and mental health services including substance abuse, when necessary, job training access, and public benefits where eligible.
•Work to effectively meet clients’ needs and resolve individual barriers through follow-up, advocacy, and collaboration with DWC staff and other community service providers.
•Process Brilliant Corners & HACLA voucher applications and maintain collaborative relationships with HACLA & Brilliant Corners.
•Maintain accurate and timely records of activities, case management notes, and services provided to each client.
•Collaborate with Mental health agencies, hospitals, and clinic settings to ensure continuity of care is developed for clients of the program.
•Advocate for best practices to ensure that clients retain housing, increase life skills, improve health and mental health.
•Provide supervision and guidance to interns and volunteers in the provision of services to clients.
•Develop and maintain relationships with clients, staff, and local service providers, participate in community meetings pertaining to services benefiting clients, and serve as a representative of DWC at community events.
•Effective crisis management skills and conflict resolutions skills
•Be an ambassador of DWC and its mission and values by participating in community meetings/events
•Provide weekly written reports to the Housing for Health Program and additional reports as assigned

Supervisory Responsibility
This position does not have supervisory responsibilities.
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. Employees must able to complete weekly home visits which require reading directions, driving, walking, and standing. Employees must perform administrative tasks such as making phone calls, navigating the computer, typing correspondence, ability to lift files, open filing cabinets, bend and stand as necessary.
This is a full-time, non-exempt position. Days and hours of work are Monday – Friday from 8:00am-4:30pm
Travel
Regular travel throughout Los Angeles County is required in this position.

Required Education, Experience, Knowledge, Skills, and Position Qualifications
Strong knowledge of the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, health issues, and older adults; Familiarity with permanent supportive housing model and evidence-based practices.
•One (1) year of experience working with homeless individuals AND have a social work/mental health related bachelor’s degree required OR a minimum of two (2) years of experience providing direct mental health services, intensive case management services, or permanent supportive housing required.
•HMIS experience preferred, with knowledge of data collection tools to track client progress.
•Excellent active listening and crisis intervention skills with ability to exercise and model good judgment.
•Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences.
•Ability to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with data management responsibilities.
•Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required.

•Ability to be flexible with hours when needed.

•Must be a CA resident and live no more than 75 miles from DWC; must be available for in-person meetings at DWC.
•Current valid California state driver’s license, comprehensive insurance, and good driving record
•Must provide documentation of a negative TB test result within the last twelve months, on first day of hire.
•You will be required to be up to date on your vaccinations against COVID-19 and provide documentation. If you are fully vaccinated and not yet eligible to receive a booster dose, then you must receive the booster dose within 30 days of becoming eligible and eligibility begins 5 months after receiving final primary dose. If you are fully vaccinated and have received a booster dose, you are considered up to date with your vaccinations. Unvaccinated individuals must receive their 1st dose before their first day of hire. Partially vaccinated (for 2-dose series) individuals must receive their 2nd dose within 30 days of being hired. The second dose of a two-dose vaccine series is due 4 weeks after the primary dose.
