Job description

About the Position:

This role aims to assist people in Los Angeles County who lack access to Housing, Mental Health, Substance Abuse treatment, and other community resources, with the goal of reducing recidivism. The Intensive Case Manager will create a supportive network of services and resources to prevent reincarceration and support long-term housing stability.

What You Will Do:

  • Interview and assess participant needs through referrals from the Office of Diversion and Reentry.
  • Conduct wellness checks at clients’ homes and provide crisis management.
  • Develop treatment plans to identify and address service needs and goals.
  • Assist with public benefits advocacy and linkage.
  • Coordinate access to mental health, substance abuse, and housing resources.
  • Support placement and case management upon client release.
  • Collaborate with housing staff, probation officers, psychiatrists, public defenders, primary care providers, and other partners to serve participants effectively.
  • Participate in inter-agency case meetings, coalitions, regional advocacy groups, and boards.
  • Maintain concise and comprehensive case records for all participants.
  • Provide accurate documentation and case notes as required.
  • Use company and personal vehicles to visit county jails and transport clients as needed.

What You Will Bring:

  • Bachelor’s degree in behavioral health or social science with at least one year of relevant experience, OR
  • The equivalent of a high school diploma with three years of administrative experience in outreach, substance abuse counseling, reentry services, and case management within a diverse population.
  • Knowledge of assessments, substance abuse counseling, reentry services, and case management.
  • Strong organizational skills and attention to detail.
  • Excellent communication skills, both written and oral.
  • Knowledge of non-profit organizations.
  • Proficiency in Microsoft Office Suite or similar software.
  • Valid driver’s license, car insurance, and access to a vehicle.
