SUMMARY:
In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team the Case Manager will provide Intensive Case Management Services (CMS) to clients recently released from jail who are experiencing homelessness. The Case Manager will assist with coordinating client’s release and help them attain permanent supportive housing in the community.
The ODR Case Manager will work with clients to provide intensive field-based case management to a diverse group of clients with complex trauma facing medical, mental health and substance use barriers. Clients remain in the program for as long as they require subsidized housing.
