Job summary

Sunnyhill, Inc. is seeking a part-time Personal Outcomes Coach to support individuals with IDD in Saint Louis County. The role emphasizes providing ongoing training on daily living skills, guided by ISP outlines, and fostering a joyful, engaging environment.

Qualifications

Energetic, compassionate communicator who can support individuals with IDD.
Strong organizational skills and attention to detail.
Ability to document progress and maintain records accurately.
Positive attitude with ability to work independently and in a team.

Responsibilities

Provide ongoing training regarding daily living skills per ISLs and ISP outlines.
Offer continuous support to participants with a friendly, joyful approach.
Maintain daily progress notes and ensure compliance with funding guidelines.
Attend meetings and communicate with participants, guardians, and providers.
Assist in developing and reviewing safety plans and maintaining safety supplies.
Represent Sunnyhill with professionalism and a sunny, engaging attitude.
