Job summary

Compass Health, Inc. is seeking a Support Coordinator in Clinton, MO to connect individuals with developmental disabilities to essential services and supports.

This full-time role runs Monday–Friday, 8:00am–5:00pm, with travel to multiple locations for assessments and ongoing support. You will develop person-centered care plans, coordinate services, and monitor provider performance to ensure quality and independence.

Qualifications

• Education requirements include a bachelor’s degree or nursing diploma/associate degree.
• Master’s degree in related fields is required.
• Valid driver’s license and agency minimum automobile coverage required.

Responsibilities

• Conduct comprehensive assessments and ongoing reassessments to identify needs and opportunities.
• Develop and update person-centered care plans reflecting goals and progress.
• Coordinate connections to services, supports, and community resources.
• Monitor service delivery for quality and alignment with goals.
• Maintain timely, accurate documentation per agency standards.
• Collaborate with interdisciplinary teams to review progress and adjust services.
• Travel to various locations to perform assessments and provide ongoing support.

Education

Bachelor’s degree

Nursing Diploma/Associate degree in nursing

Master’s degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy or closely related field
