Compass Health Network seeks a Community Support Specialist - Youth to provide in‑home and community services to youths and families in Clinton, MO. You will develop and implement Wellness Plans, support caregivers, and coordinate with schools and healthcare partners to promote resilience and safe outcomes.

The role requires a high school diploma or GED and may require a bachelor’s/associate degree in a human services field; driving in the community is essential.

Formación

• High School/GED required.
• Associate’s or Bachelor’s degree in a human service‑related field preferred.
• Valid driver’s license and ability to travel in the community.

Responsabilidades

• Deliver strengths‑based, holistic services to youth and families based on individualized Wellness Plans.
• Support family engagement and participation in the treatment process.
• Provide coaching, skill‑building, and access to community resources.
• Collaborate with school staff, healthcare providers, and other child‑serving systems.
• Offer crisis support and assist in developing safety plans.
• Communicate regularly with the client’s integrated care team.
• Document services accurately and timely in agency records.
• Travel within the community to meet clients in homes or communities.
• Flex schedule, including some evenings and weekends.
