Job summary

CPM is seeking a compassionate Day Support Specialist to help individuals build skills, relationships, and independence. You will create engaging opportunities that encourage participation, personal growth, and community connection.

The role requires initiative, clear communication, and a patient, respectful approach to support individuals in daily activities, transportation, and documentation of progress.

Qualifications

• High school diploma or GED required.
• Valid driver's license and reliable transportation.
• DDS medications certification, First Aid, CPR, and PMT certifications within six months of hire.
• Experience supporting individuals with intellectual or developmental disabilities preferred.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Actively engage individuals in meaningful activities within program and community settings.
• Teach, assist, and supervise individuals during activities, employment opportunities, and skill-building exercises.
• Plan community outings and provide individuals with multiple activity options.
• Assist with personal care while maintaining dignity, privacy, and safety.
• Support individuals in achieving goals identified in their ISP.
• Accurately document ISP objectives, attendance, behavioral data, participation, and progress.
• Maintain organized and current case records.
• Report behavioral, medical, or routine changes to the supervisor or team.
• Safely transport individuals between locations and work sites.
• Maintain vehicle logs and ensure vehicles are clean and safe.
