Qualifications

• Bachelor’s degree in social work or equivalent human services degree or RN license.
• Valid driver’s license, reliable vehicle, and proof of insurance.
• Experience with individuals with developmental or intellectual disabilities preferred.

Responsibilities

• Independently manage a professional caseload in a geographic area.
• Conduct comprehensive assessments with individuals, families, guardians, and providers.
• Develop, implement, and update individualized, person-centered support plans.
• Coordinate, authorize, and monitor services per plans and funding requirements.
• Prepare and review purchase-of-service authorizations and related documentation.
• Maintain accurate, timely, compliant case documentation using electronic systems.
• Serve as liaison with provider agencies and state partners.
