Job summary

The Arc of Union County is seeking a Service Plan Coordinator to develop, implement, and monitor individualized service plans for individuals with intellectual and developmental disabilities.

Under the direction of the Director, Quality, Compliance & Planning, you will coordinate person-centered services, advocate for recipients, and ensure compliance with DDD, DVR, NJ 10.44A, DCF, and CARF standards to support independence and community integration.

Qualifications

• Bachelor’s degree in Social Work, Special Education, Human Services, Rehabilitation, or Health-Related Sciences is required, with IDD training preferred.
• At least 5 years of experience in social services or with individuals with IDD.
• Certification as ASQ (American Society for Quality) or CQA (Certified Quality Auditor) required within 6 months of hire.
• Knowledge of NJ DDD day habilitation, DVR, and NJ 10.44A residential services; familiarity with DCF and CARF standards for IDD.

Responsibilities

• Develop, implement, and monitor individualized service plans for service recipients.
• Promote coordination of person-centered services through interagency collaboration and advocacy.
• Ensure compliance with state and federal eligibility requirements and CARF standards.
• Monitor service recipients to ensure timely, individualized support and optimal well-being.
