Arc of Onondaga in New York is seeking a Centralized Plan Coordinator to develop and coordinate person-centered service plans that reflect each individual's goals, strengths, and aspirations. You will partner with individuals, families, and interdisciplinary teams to create, implement, monitor and update plans, ensuring quality outcomes and compliance with regulations.

The role emphasizes advocacy, collaboration, and accurate documentation to maintain up-to-date plans and effective service

Formación

• Bachelor's degree in Human Services or related field.
• At least one year of experience supporting individuals with developmental disabilities.
• Valid New York State driver's license with driving record meeting agency requirements.

Responsabilidades

• Develop, coordinate, and maintain person-centered service plans.
• Collaborate with individuals, families, care providers, and interdisciplinary teams to identify goals and support needs.
• Monitor implementation of individualized plans and ensure services align with established goals.
• Maintain accurate, timely, and compliant documentation.
• Coordinate plan reviews and updates as individuals' needs and goals evolve.
• Ensure services meet agency policies as well as state and federal regulations.
• Advocate for individuals to ensure their voices, choices, and preferences remain central to the planning process.
• Build positive relationships with internal teams and community partners to support successful outcomes.

Educación
