Job summary

United Counseling Service (UCS) seeks a Service Coordinator to develop, implement, and monitor services for individuals with intellectual or developmental disabilities. You will coordinate care plans, liaise with providers, and conduct home visits to ensure compliance and quality of life.

Qualifications

• High School Diploma or equivalent with relevant Service Coordination experience.
• At least one year of experience in Developmental Services or a Bachelor’s degree in Human Services or a related field preferred.
• (QDDP) Qualified Developmental Disabilities Professional preferred; increases pay.
• Windows skills, typing, navigation, and e-mail usage are required.
• Strong interpersonal, communication, and writing skills are desired.

Responsibilities

• Facilitates and documents the person-centered planning process and ISA implementation.
• Completes necessary documentation in a timely fashion; engages clients regularly.
• Reviews ISA expectations and data collection to ensure proper service delivery.
• Coordinates services with physicians, therapists, and other providers as needed.
• Conducts home visits monthly and adheres to state regulations.
• Liaises between agency personnel and other providers to develop services.
