The Arc of Howard County is seeking a Planning Specialist to provide quality planning and technical support for the Person-Centered Planning (PCP) process and LTSS billing. The role includes basic case management oversight and coordination with teams to ensure compliant service delivery.

Qualifications

• A bachelor's degree in a relevant field is required.
• Knowledge of Code of Maryland Regulations (COMAR) & Home and Community-Based Services (HCBS) Final Rule, waiver and service definitions and regulations, guidelines, procedures & philosophy required.
• Three years of experience in the field of IDD is preferred.
• Person-Centered planning development and implementation experience is strongly preferred.

Responsabilités

• Support the PCP Process with coordination of community living, meaningful day, CDS, employment, supported living or other services.
• Lead and train agency staff to support person-centered initiatives, plans and outcomes.
• Manage PCP scheduling and ensure representation of The Arc staff with Coordinators of Community Services.
• Assist with documentation training in ICM as needed.
• Review draft and final person-centered plans for accuracy as it relates to the person.
