Job summary

Arc of Onondaga in New York is seeking a Centralized Plan Coordinator to develop and coordinate person-centered service plans that reflect each individual's goals, strengths, and aspirations. You will partner with individuals, families, and interdisciplinary teams to create, implement, monitor and update plans, ensuring quality outcomes and compliance with regulations.

• Bachelor's degree in Human Services or related field.
• At least one year of experience supporting individuals with developmental disabilities.
• Valid New York State driver's license with driving record meeting agency requirements.

About the Role

The Centralized Plan Coordinator partners with individuals receiving services, their families, and interdisciplinary teams to create, implement, monitor, and update individualized service plans. This position ensures services are delivered in accordance with person-centered planning principles, agency standards, and regulatory requirements while promoting quality outcomes for the people we support.
