Job summary

InspiredOptions Inc. is seeking an experienced PCP Coordinator to support high-quality, person-centered services for individuals with intellectual and developmental disabilities in Maryland.

The role focuses on coordinating PCPs, SIPs, authorizations, and team collaboration across CCS, families, nursing, and billing to ensure care aligns with approved plans. The ideal candidate has 3–5 years in developmental disabilities, strong knowledge of Maryland DDA processes, and experience with EHR

Qualifications

• Bachelor's degree in human services or related field; equivalent experience considered.
• 3–5 years in developmental disabilities or human services roles.
• Experience with Person-Centered Plans, SIPs, authorizations, documentation, and interdisciplinary coordination.
• Experience using EHR or case management systems.

Responsibilities

• Manage annual PCP calendar, meetings, revisions, and interdisciplinary team meetings.
• Review PCPs for outcomes, services, staffing, health and safety needs.
• Develop and maintain SIPs aligned with PCPs and authorizations.
• Translate PCP outcomes into measurable implementation steps for DSPs.
• Track participant outcomes, goals, progress, and required documentation.
• Review service notes and documentation for gaps in implementation.
• Monitor service authorizations, hours, units, dates, and changes.
• Collaborate to maintain accurate service and authorization data in CIMS.
• Coordinate DSAT information based on participant needs.
• Work with nursing staff to incorporate HRST and medication considerations.
• Assist new admissions by reviewing PCPs, authorizations, and needs.
• Train DSPs and staff on PCP outcomes, SIPs, and documentation standards.
• Conduct quality assurance reviews and audits of PCPs, SIPs, and records.
• Identify discrepancies and coordinate resolutions with stakeholders.
• Maintain confidential, audit-ready participant records.
• Support readiness for billing and regulatory reviews.
• Assist with updates after hospitalizations or major changes.
