Job summary

The Arc of Union County, Inc. in Springfield, NJ is seeking a Service Plan Coordinator to develop, implement, and monitor individualized service plans for recipients in social, healthcare, or community-based settings.

Under the guidance of the Director, Quality, Compliance & Planning, you will coordinate interagency services, advocate for participants, and ensure timely, coordinated supports while adhering to DDD, DVR, NJ 10.44A, DCF, and CARF standards.

Qualifications

• Bachelor's degree in relevant field required; IDD training preferred.
• Minimum 5 years in social services with IDD experience.
• ASQ or CQA certification required within six months of hiring.
• Knowledge of DDD day habilitation, DVR, NJ 10.44A residential services; DCF and CARF standards.
• Experience with Medicaid waiver programs or other community-based supports; person-centered planning.

Responsibilities

• Develop, implement, and monitor individualized service plans for recipients.
• Coordinate interagency collaboration and advocacy to meet needs and goals.
• Ensure timely, coordinated services and regulatory compliance.

Education

Bachelor's degree in Social Work / Special Education / Human Services / Rehabilitation / Behavioral Sciences / Health-Related Sciences

ASQ (American Society for Quality) or CQA (Certified Quality Auditor) certification within six months of hire
