Job summary

The Arc Erie County New York is seeking a dedicated Program Coordinator to support individuals by developing and coordinating person-centered habilitation plans, promoting growth, independence, and meaningful outcomes.

In this full-time role, you will meet with individuals to develop goals, update plans per agency standards, and ensure proper implementation. You will document progress, collaborate with staff and service providers, and a training program will be provided for the right candidate.

Qualifications

• Bachelor’s Degree in Psychology, Human Services, or related field (required).
• Minimum of one year experience working with individuals with intellectual and developmental disabilities.
• Strong written communication and organizational skills.
• Ability to work independently and manage multiple priorities.
• Clean New York State Driver’s License.

Responsibilities

• Meeting with individuals to support goal development and promote personal growth and independence.
• Developing and updating habilitation plans in line with agency standards and regulatory requirements.
• Ensuring plans are implemented and aligned with individual needs and outcomes.
• Communicating plan updates to relevant staff and departments.
• Maintaining clear documentation and records.
• Supporting collaboration between individuals, staff, and service providers.
• Training will be provided for the right candidate.
