Qualifications

• High school diploma or GED required.
• One year experience with individuals with IDD preferred.
• Reliable transportation and willingness to work in a drug-free workplace.

Responsibilities

• Develop and implement Individual Person-Centered Plans and Individual Support Plans.
• Provide daily instruction on plan goals and activities.
• Evaluate progress and prepare progress reports.
• Attend interdisciplinary team meetings and participate in plan revisions.
• Serve as a driver or driver's aide for transportation.
• Maintain a safe environment and assist with daily needs.
• Coordinate community experiences and inclusive activities.
• Complete required reports and attend staff trainings.
