Qualifications

• A high school diploma or GED is required.
• One year of experience working directly with individuals with intellectual/developmental disabilities is preferred.

Responsibilities

• Develop and implement Individual Person-Centered Plans/Individual Support Plans.
• Provide daily instruction in plan goals and activities.
• Evaluate progress and prepare progress reports on individual outcomes.
• Attend interdisciplinary team meetings and aid in plan revisions.
• Serve as a driver or driver's aide for program transportation and community experiences.
• Develop new or revised client outcomes and program activities with input from individuals.
