Job summary

Columbus House, Inc. in New Haven, CT is seeking a dedicated PSH Case Manager to support adults experiencing homelessness by securing housing and linking to health, mental health, and benefits services.

You will collaborate with landlords and social service providers, maintain detailed case documentation, and participate in regular meetings while upholding person-centered, confidential service delivery.

Qualifications

• Excellent organizational and time management skills with a deadline-oriented mindset.
• Strong written and oral communication skills employing the English language.
• Proficient in Microsoft Office and other common desktop and online applications.
• Knowledge of and ability to effectively navigate substance abuse, mental health, and social services networks.
• Familiar with criminal justice barriers and able to engage, motivate, and support the individuals serviced.
• Knowledgeable of crisis intervention, boundaries, confidentiality, and harm reduction principles.
• Able to meet documentation requirements within set timeframes.
• Valid drivers’ license with a good driving history.

Responsibilities

• Assist clients with finding, securing, and maintaining housing.
• Provide linkage to resources for clients at risk of eviction and connect to community services.
• Advocate for clients and support self-advocacy.
• Coordinate with landlords, property managers, and other agencies.
• Maintain confidentiality and professional boundaries; attend meetings and document work.
• Complete required assessments and documentation in electronic and written forms.
