Job summary

LifeWorks in Austin, TX is seeking a dedicated Case Manager for our Permanent Supportive Housing (PSH) program. The role provides housing navigation and stabilization services to individuals experiencing homelessness, coordinating with healthcare partners to support client needs.

You will also maintain client files, enter data, and complete required documentation while empowering clients to pursue self-defined housing goals and access community resources.

Qualifications

• Bachelor's degree in social or behavior sciences or administrative field.
• Minimum 2 years related experience; or equivalent.
• Lived experience of homelessness is a plus and may count toward related qualifications.

Responsibilities

• Provide client-centered, healing-centered case management to diverse individuals experiencing homelessness or related challenges.
• Identify and leverage clients' strengths to support self-defined goals.
• Document and maintain timely, accurate case records and progress notes.
• Assist clients with day-to-day needs and life-skill development, including budgeting and transportation.
• Ensure consent and program requirements are understood and followed.
• Engage with the community and partners to advocate for client needs and referrals.
