Job summary

The People Concern is seeking a Case Manager - ODR Scattered Site to provide Intensive Case Management Services (ICMS) for clients recently released from jail and experiencing homelessness. You will coordinate releases, link to health, housing, and community resources, and support long‑term housing stability through a trauma‑informed, harm‑reduction approach.

You will work in The Nest location in downtown Los Angeles with a Monday–Friday schedule and a full‑time non‑exempt status, serving up to

Qualifications

• Bachelor's degree preferred or two years' case management experience with homelessness.
• Clean CA driver’s license, reliable car, auto insurance, and good driving record.
• Able to pass a background check for jail clearance.
• Able to transport clients in a vehicle.
• Demonstrated knowledge of case management.
• Able to work in a harm-reduction model with clients facing multiple barriers.
• Skill in non‑violent crisis intervention.
• Computer literate; basic MS Word, Outlook, and Excel.
• Detail oriented with strong time management, organizational, written and verbal skills.
• Able to obtain CPR/First Aid certification.

Responsibilities

• Maintain a caseload of clients with complex needs and barriers.
• Document client contacts in the agency database and HIPAA‑compliant systems.
• Coordinate appointments, transportation, and follow‑up services for health and housing resources.
• Develop client biopsychosocial assessments and update quarterly.
• Collaborate with clients to create and revise goal plans for housing stability.
