Job summary

All Roads is seeking a Permanent Supportive Housing Case Manager to support homeless adults in Boulder, CO. The role focuses on intensive case management to help clients move into stable housing and maintain it, with emphasis on coordination of services and housing vouchers.

The position requires a bachelor’s degree or equivalent certification, strong client communication, and the ability to travel to residences. Salary commensurate with experience and full benefits are provided.

Qualifications

• Bachelor’s degree, 3 years of relevant experience, or Qualified Behavioral Health Assistant Certification required.
• Minimum 6 months’ experience in homeless services.
• BA in social work, psychology, or related field preferred.
• Valid driver’s license is required for travel to clients’ residences.

Responsibilities

• Assist clients in obtaining and maintaining housing and income supports.
• Ensure HIPAA privacy and security standards in all communications and records.
• Help clients connect with health, mental health, and substance use services.
• Document case notes for Medicaid billing with accuracy and timeliness.
• Refer clients to community programs and assist with paperwork and vouchers.
• Maintain relationships with landlords, voucher administrators, and providers.
