We are seeking a compassionate and motivated Occupational Therapist (OT) to join a 126-bed skilled nursing facility in Fremont, CA. This permanent position offers a consistent Monday–Friday schedule with no weekend rotation, making it an excellent opportunity for clinicians who value both professional growth and work-life balance.
In this role, you'll work directly with patients to help improve their functional independence, participate in meaningful daily activities, and achieve individualized therapy goals. You'll also collaborate with an interdisciplinary team to deliver high-quality, patient-centered care.

Perform comprehensive occupational therapy evaluations and assessments
Develop individualized treatment plans based on patient needs, abilities, and goals
Provide skilled OT interventions to improve functional independence and quality of life
Address activities of daily living, mobility, coordination, strength, and other functional needs
Monitor patient progress and modify treatment plans when appropriate
Maintain accurate and timely clinical documentation
Collaborate with nursing, rehabilitation, and other interdisciplinary team members
Educate patients, families, and caregivers regarding therapeutic strategies and techniques
Participate in care planning and contribute to overall patient outcomes
Follow facility policies, clinical standards, and applicable regulatory requirements
Graduate of an accredited Occupational Therapy program
Current California Occupational Therapist license or eligibility for licensure
Strong communication, organizational, and clinical skills
Commitment to patient-centered care
Ability to work effectively within an interdisciplinary team
SNF experience is preferred but not required
