Conduct comprehensive occupational therapy evaluations and assessments
Develop individualized treatment plans based on patient needs and goals
Provide skilled OT interventions to improve functional independence and quality of life
Address activities of daily living, mobility, coordination, strength, and other functional limitations
Monitor patient progress and modify treatment plans as needed
Accurately document evaluations, treatments, progress, and outcomes
Collaborate with nursing, rehabilitation, and other members of the interdisciplinary team
Educate patients, families, and caregivers on therapeutic techniques and strategies
Participate in care planning and contribute to positive patient outcomes
Maintain compliance with facility policies, clinical standards, and applicable regulations
Graduate of an accredited Occupational Therapy program
Current California Occupational Therapist license or eligibility for licensure
Strong clinical, communication, and organizational skills
Commitment to compassionate, patient-centered care
Ability to work collaboratively with an interdisciplinary team
SNF experience is preferred but not required
