Job summary

Talented Medical Solutions seeks a dedicated Occupational Therapist to join a 99-bed skilled nursing facility in Hayward, CA. This permanent, full-time role offers a Monday–Friday schedule with 35 hours per week and no weekend rotation, promoting a healthy work-life balance while you help patients regain independence.

You will evaluate patients, develop individualized plans, and provide interventions to improve daily living activities, mobility, and overall quality of life in a supportive SNF

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license or eligibility is required or pursued.
• Strong clinical, communication, and organizational skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs and goals.
• Provide skilled OT interventions to improve functional independence and quality of life.
• Address activities of daily living, mobility, coordination, strength, and other functional limitations.
• Monitor patient progress and modify treatment plans as needed.
• Accurately document evaluations, treatments, progress, and outcomes.
• Collaborate with nursing, rehabilitation, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers on therapeutic techniques and strategies.
• Participate in care planning and contribute to positive patient outcomes.
• Maintain compliance with facility policies, clinical standards, and applicable regulations.
