We are seeking a compassionate and motivated Occupational Therapist (OT) to join a small skilled nursing facility in Willows, CA. Evaluate patients' functional abilities and occupational therapy needs
Develop individualized treatment plans based on patient goals and clinical needs
Provide skilled occupational therapy interventions to improve mobility, independence, and activities of daily living
Monitor patient progress and adjust treatment plans as appropriate
Document evaluations, treatments, progress, and outcomes accurately and timely
Collaborate with nursing, therapy, and other members of the interdisciplinary care team
Educate patients, families, and caregivers on appropriate therapeutic techniques and strategies
Participate in care planning and contribute to positive patient outcomes
Follow facility policies, clinical standards, and applicable regulatory requirements
Graduate of an accredited Occupational Therapy program
Current California Occupational Therapist license or eligibility for licensure
Strong clinical, communication, and organizational skills
Commitment to providing compassionate, patient-centered care
Ability to work effectively as part of an interdisciplinary team
SNF experience is a plus, but motivated clinicians with other clinical experience are welcome to be considered
