Job summary

Talented Medical Solutions is seeking a compassionate Occupational Therapist for a small SNF in Willows, CA. This permanent, full-time role offers a Monday–Friday schedule with no weekend rotation, enabling meaningful patient care while preserving work-life balance.

You will work closely with a collaborative care team to support patients in improving functional independence and quality of life in a smaller SNF setting.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license or eligibility.
• Strong clinical, communication, and organizational skills.
• Commitment to providing compassionate, patient-centered care.
• Ability to work effectively as part of an interdisciplinary team.
• SNF experience is a plus.

Responsibilities

• Evaluate patients' functional abilities and OT needs.
• Develop individualized treatment plans based on goals and clinical needs.
• Provide skilled OT interventions to improve mobility, independence, and ADLs.
• Monitor patient progress and adjust treatment plans as appropriate.
• Document evaluations, treatments, progress, and outcomes accurately and timely.
• Collaborate with nursing, therapy, and other care team members.
• Educate patients, families, and caregivers on therapeutic techniques.
• Participate in care planning to contribute to positive outcomes.
• Follow facility policies, clinical standards, and regulatory requirements.
