Job summary

Breaking Ground in New York City seeks a Case Manager, Permanent Housing to provide direct services to a caseload of clients, conduct home visits, assess needs, and link clients to benefits, housing support, and community resources to sustain permanent housing.

The role requires a Bachelor's Degree or equivalent, experience with homeless adults, harm reduction, strong writing and communication skills, and a valid driver’s license. This full-time position is based on-site in NYC.

Qualifications

• Bachelor's Degree or equivalent work experience.
• Two years of experience with homeless adults and substance abuse preferred.
• Experience with harm reduction and motivational interviewing preferred.
• Valid driver’s license for NY/NJ/CT/PA preferred.
• Excellent writing and communication skills.

Responsibilities

• Provide direct services to a caseload of~15 clients, including home visits and assessments.
• Assist clients with benefits and entitlements to help maintain housing.
• Connect clients to psychiatric and medical services as needed.
• Coordinate group activities and link to community resources.
• Maintain records in compliance with agency and state standards.
• Meet weekly with clients to identify needs and develop service plans.
• Support self-administration of medications per policy.
• Intervene in crisis situations and provide crisis support.
• Assist with documentation for recertification and housing stability.
