• Performs holistic assessments of patients across physical, psychosocial, spiritual, and education needs.
• Develops and revises a multidisciplinary plan of care with measurable goals.
• Communicates care plans using SBAR and handoff processes.
• Coordinates discharge planning and appropriate referrals.
• Delivers ongoing patient and family education aligned with the care plan.
• Responds to emergencies and upholds safety and infection control standards.
