Qualifications

Responsibilities

• Assess patient needs and plan care with the care team.
• Implement and coordinate nursing care per plan.
• Document assessments and interventions in the chart.
• Evaluate patient responses and adjust care.
• Collaborate with physicians and other health professionals.
• Provide patient and family education.
