Job summary

The Christ Hospital Health Network is seeking a registered nurse to provide comprehensive, evidence-based care within its Professional Practice Model. The RN will assess, plan, implement and evaluate patient care, partnering with patients and families to promote outcomes.

Responsibilities include holistic assessments, care coordination with the healthcare team, and patient/family education. Licensure is required; BSN within five years is preferred, with ongoing professional development

Qualifications

• Must be a licensed RN with current state licensure.
• BSN within 5 years of hire is preferred.
• Demonstrates knowledge of patient-centered care and evidence-based practice.

Responsibilities

• Perform holistic patient assessments across physical, psychosocial, spiritual, and education needs.
• Develop and implement the plan of care in collaboration with the patient and family.
• Coordinate with multidisciplinary team to ensure safe, effective care and timely discharge planning.
• Provide ongoing patient and family education and participate in performance improvement activities.
