Responsibilities

• Assess physical, social, and psychological status and information from the care team.
• Develop an evidence-based, individualized plan of care with patient and family.
• Coordinate care, delegate tasks, and allocate time and resources.
• Implement care according to plan and licensure; ensure safety and quality.
