Job summary

Qualifications

• Active RN license in state of practice; compact transfer within 60 days where applicable.
• BSN required; new hires must meet BSN requirement as of 4/27/2025.
• BLS certification for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status and information from interdisciplinary team; collect and evaluate data.
• Plan and develop an individualized care plan with patient and family involvement, including education and referrals.
• Implement care by coordinating the team and resources; ensure timely documentation and adherence to licensure.
• Evaluate patient responses and revise the plan as needed to achieve outcomes.
