Job summary

You will work with clinical teams and social service agencies to guide prenatal, delivery, and postpartum services, provide screening, education and care coordination, and ensure timely navigation to appointments with follow-up reporting.

Qualifications

• Bachelor’s degree in social sciences, health education or related field required.
• Four years of experience in health education, public health, social work or case management may substitute education.
• Community Health Worker certification preferred.
• Spanish fluency is preferred.

Responsibilities

• Serve as liaison between health system providers and community resources to improve maternal health outcomes.
• Collaborate with clinical care teams and social service agencies to guide prenatal, delivery and postpartum services.
• Provide screening services, education, support, and patient care coordination.
• Disseminate information, navigate patients to medical appointments, and follow up on health needs.
• Track patients, perform data entry and generate reports.
