Job summary

Qualifications

Responsibilities

• Coordinate an individualized plan of care with an interdisciplinary team.
• Assist scheduling and linkage to services across the continuum of care.
• Educate patients and families about disease processes and treatment plans.
• Advocate for patients to remove barriers to care.
• Coordinate referrals and transitions to specialty services to ensure timely care.
• Monitor patient outcomes and participate in quality improvement activities.
