• Work with a clinical care team and social service agencies to guide patients through prenatal, delivery, and postpartum services.
• Provide screening, education, support, and care coordination; disseminate information and navigate patients to medical appointments.
• Follow up on health care needs; track patients and contribute to data entry and reporting.
