Job summary

Baltimore County is seeking a part-time Health Navigator in Clinical Services/Maternal Child Health. The role supports perinatal navigation for pregnant and postpartum women, coordinates with partners, and ensures proper documentation.

Qualifications

Responsibilities

• Provides care coordination, health education, identifies resources, and conducts telephone and in-home perinatal navigation services for pregnant and postpartum women.
• Conducts interviews and screenings for high-risk pregnant and postpartum women to identify needs and link to resources.
• Coordinates with community partners to link clients with available programs and services.
• Completes documentation, encounter forms and data tracking.
• Maintains liaison with public and private agencies.
