University Health seeks a liaison for the Baby Bexar Healthy Families Community-Based Perinatal Health Initiative to connect providers with community resources and improve maternal health outcomes for patients. You will work with a clinical care team and social service agencies to guide patients through prenatal, delivery and postpartum services.

Responsibilities include screening, education, support, care coordination, and ensuring timely information dissemination and follow-up.

Qualifikationen

• Bachelor’s degree required in social sciences, health education or related field.
• Four years of experience in health education, public health, social work and/or case management may be considered in lieu of the Bachelor’s degree.
• Community Health Worker certification preferred.
• Spanish fluency is preferred.

• Serve as liaison between health system providers and community resources to improve maternal health outcomes for University Health patients.
• Collaborate with clinical care teams and social service agencies to guide prenatal, delivery and postpartum services.
• Provide screening, education, support and patient care coordination.
• Ensure information is disseminated, navigate patients to medical appointments and follow-up on health care needs.
• Engage in patient tracking, data entry and reporting for program monitoring.
