Qualifications

• Valid Wisconsin driver license and car insurance.

Responsibilities

• Provide case management to prenatal patients from onset to 8 weeks postpartum.
• Perform risk assessments at pregnancy start and as needed.
• Refer patients to providers or community agencies as needed.
• Educate about labor readiness, newborn care, contraception options.
• Ensure timely NOP documentation for prenatal services.
• Comply with Wisconsin PNCC and CCC requirements.
• Conduct newborn risk assessments and home visits as relevant.
• Schedule follow-ups for newborn with pediatrician.
• Support breastfeeding through education by various means.
• Facilitate natural family planning education as directed.
• Assist patients with Stork's Nest program tracking.
• Assist perinatal case managers as needed.
