Qualifications

Responsibilities

• Provide case management to prenatal patients from onset to 8 weeks postpartum.

• Ensure timely NOP documentation for prenatal services.

• Facilitate natural family planning education as directed.
• Assist patients with Stork's Nest program tracking.

JOB RESPONSIBILITIES:

1. Provide case management to all patients of Sixteenth Street Community Health Centers beginning at onset of prenatal care until 8 weeks postpartum. Responsibilities include but are not limited to:
   1. Performance of risk assessments at the beginning of each pregnancy and as necessary
   2. Refer patients to Sixteenth Street provider or community agencies/specialists as needed
   3. Health and breastfeeding education:
      1. Assess prenatal patients' knowledge and provide education on labor readiness and newborn care
      2. Educate patients on contraception options including natural family planning
   4. Timely Submission and documentation on Notification of Pregnancy (NOP) for all patients receiving prenatal services with HMOs
      1. Track reimbursement for all NOPs submitted and notify WW Team Lead if reimbursement is not received timely
2. Comply with the State of Wisconsin requirements for Perinatal Care Coordination (PNCC) as outlined in the Sixteenth Street policies and procedures and the PNCC Handbook
3. Comply with the State of Wisconsin requirements for Child Care Coordination Manager (CCC). Responsibilities will include
   1. Performing a risk assessment on newborns when indicated
   2. Referral to appropriate agencies/specialist as needed
   3. Health education to parents of the pediatric patients
   4. Home visits as appropriate
   5. Breastfeeding/ feeding education and support
4. Maintain appropriate and timely documentation patient records for PNCC and OB Medical Home (OBMH) programs
5. Conduct home visits for clinic patients as required by PNCC and OBMH guidelines and as requested by Sixteenth Street providers
6. Conduct a minimum of one postpartum home visit to review patient self-care, newborn care, family planning methods, and provider instructions. Visit may be conducted by phone or at one of the Sixteenth Street locations if patient desires, if medically indicated, or if patient resides outside of Milwaukee County
7. Support breastfeeding success by performing prenatal and postpartum education by phone, clinic visit, or home visit
8. Facilitate breastfeeding classes and support groups
9. Facilitate Natural Family Planning education to patients as required or delegated by Lactation Counselor
10. Assist Patients with accessing the Stork's Nest
    1. Track and document individual patient points and items exchanged
    2. Track and document Stork's Nest inventory patient points and item exchanged
11. Ensure appropriate scheduling of follow up care for newborn with Sixteenth Street pediatrician
12. Assist perinatal case managers as needed

QUALIFICATIONS:

1. High School Diploma or GED
2. Completion of lactation consultant training program, certification highly recommended
3. Knowledge of Sixteenth Street Community Health Center service community
4. Ability to relate well to people from diverse ethnic and cultural backgrounds.
