Job summary

Native American Health Center, Inc. seeks a Comprehensive Perinatal Health Worker to support access to high-quality perinatal care in Oakland. You will coordinate care under CPSP, process referrals, manage medical records, and schedule appointments.

The role is full-time (40 hours/week), non-exempt, reporting to the Program Manager I, Perinatal, within the Medical department. Strong teamwork and documentation are essential.

Responsibilities

• Provide new member orientations for perinatal members and CPSP assessments.
• Coordinate care with the team; identify risk levels and referrals.
• Process perinatal referrals with Referral Coordinators and schedule external appointments.
• Track referrals and follow up to ensure attendance; schedule OB ultrasound, NST, labs.
• Ensure each member has and follows an ICP.
• Resolve barriers to care, including transportation and interpretation needs.
• Keep documentation and billing codes current and accurate.
