Job summary

Polaris Healthcare is seeking compassionate Per Diem UAS Assessment RNs to perform in-home evaluations across Suffolk County, NY. You will assess medical, functional, cognitive, and social needs to determine home care eligibility.

Ideal candidates have an active NY RN license, UAS-NY certification (or willingness to obtain), and at least 1 year of nursing experience. The role offers flexible scheduling and travel within assigned areas.

Qualifications

Active New York State Registered Nurse (RN) license.
Current UAS-NY Certification strongly preferred (or willingness to obtain).
Minimum of one year of nursing experience.
Home Health/MLTC/Case Management/UAS experience preferred.

Responsibilities

Conduct comprehensive UAS-NY assessments in patients' homes throughout Suffolk County, NY.
Evaluate patients' physical, cognitive, psychosocial, and environmental needs.
Complete accurate documentation within the UAS-NY system and EMR.
Develop objective clinical assessments to guide care planning and services.
Educate patients and families about the assessment process and available home care services.
Collaborate with interdisciplinary care teams and providers to ensure continuity of care.
Maintain compliance with NYS DOH regulations and agency policies.
Travel throughout assigned service areas with a flexible schedule.
